I am going to give this form to our family physicians, with a cover letter so they under stand my intent is to get the ability to give informed consent, not to be offensive. We'll see what they say.

Ok here is my question. Yes I think we over medicate our kids, but what does one do if you want your kid to go to public school and they require full vacination? My oldest son is starting kindergarden in the fall and he requires full vacination. He is curent on all his shots, but I don't want him to get any more.

What a great form! I would like a similar form in hand now to present to any doctor at any time who tells me I need ANY prescription drug. My partner recently went to a Kaiser center asking for help in detoxing from alcohol. He was prescribed a medication called Gabapentin...He was told it would help his anxiety during the withdrawal. He was told it was a mild dose and safe.

But serious side effects came on, including loss of equilibrium, loss of appetite, loss of short term memory...Worse of all, he ended up with a severe liver inflammation,even though his previous blood enzyme levels were not so bad. Now his levels are alarming and doctors now say it could take him up to one year to see if he recovers hopefully without any permanant scarring on his liver.

It was only afterwards we discovered the truth on the internet -- that the drug is not even FDA approved for that purpose. It is often prescribed for off label uses and there is basically not a thing you can do about that. Unless of course someone dies and a lawyer may then want to assist.

I have no doubt that if this sort of form had been presented, he never would have been given a drug an anti-seizure drug that is really for epilepsy.

This incident was proof to us that you cannot even trust the so called experts of a drug detox center...as this place did not even monitor my boyfriend while he was taking the drug. Instead they ignored our statements about his reactions and doubled his dose! Talk about total negligence!

We are hoping that a recent Supreme Court decision will help people in such situations to be able to directly sue drug companies in the future for the criminal way they have convinced doctors all over to prescribe drugs in this manner.

There is an excellent book out entitled Vaccine Safety Manual: For concerned families and health practitioners by Neil Miller. Very well researched and cited. It details information and risks in quite a few vaccines and offers information for people wishing to opt out of this particular insanity.

Any doctor presented with the "Physician's Warranty of Vaccine Safety" will read perhaps the first ten lines and then quit. They don't have time during office visits to read something like this. S/he may read the rest later, but the patient would never know.

It's really not sensible to try to educate your child's doctor about the dangers of immunication during an office visit. They either know about it or they don't. The better way is to choose the correct physician beforehand --find one that is patient-friendly or who agrees with the kind of treatment you want, before the child is even born. We shop around for cars, the best deal on groceries, the safest bedding and infant car seats. The same comparative shopping should certainly be applied to something as important as your child's doctor. Don't buy bad products!

Most doctors would like to comply with patients' wishes in most treatments, but they are expected to adhere to "standards of care." Giving your doctor a chance to refrain from immunizing or to wait (till never) due to "allergies to eggs in several close family members" and/or "known history of severe reactions to MSG in close family members" or something similar would give the doctor the justification s/he can then add to the file if they desire. That way, the MD is covered --they attempted to provide the standard of care, but for a good reason, they could not completely follow that "standard."

If your MD will not respect your wishes in this matter, as in any other treatment, it's a waste of time to argue. Simply move on. Vote with your feet, don't be bullied --the same doctor who will bully a patient in this matter will likely do in other matters also. That doctor is too opinionated and would not be a good choice for any treatment --just leave and don't go back.

The religious exemption works perhaps best of alll --it's not required to specify any specific religion --and school systems must allow non-immunized religious-exempt children to attend school.

I WOULD LIKE TO KNOW WHY VACS HAVE TO BE FILLED WITH POISON??? IS THERE ANY REASON WHY THEY CANNOT BE MADE IN A HOMEOPATHIC TYPE OF FORM...LIKE SUGAR PILLS???? OR SIMPLY WITHOUT THE POISON IF THEY HAVE TO BE INJECTED...THERE MUST BE SAFE PRESERVATIVES.

I AM NOT AGAINST THE IDEA OF VACS IN GENERAL IT IS NOT A BAD IDEA BUT IF IT IS TO BE USED TO DUMB DOWN AND POISON PEOPLE THEN IT IS EVIL AND SINISTER AND I CANNOT ABIDE BY THAT...I WOULD PREFER TO TAKE MY CHANCES ON THIS WHEEL OF LIFE WITHOUT SELLING MY SOUL TO THE AMA...AND USA GOV! I DON'T UNDERSTAND WHY I NEVER HEAR OF ANYONE MAKING A HEALTHIER VACCINATION.

I love the doctor's form! I wish I had had it when that so-called "specialist" diagnosed my son with autism 5 years ago. He wanted to put my boy on some guinea-pig drug that hadn't even been cleared for the correct dosage in a child as little as my boy at the time. Sicko! I've been researching vaccines and their harmful effects for years now. I wish I had known better back when my son was born. I was so stupid! Blind, pathetic, patriotic faith. When that "specialist" diagnosed my boy, he gave me no explanation as to what autism was, no literature, but a phone number and literature on his guinea-pig trial of a anti-anxiety drug used in adults. He then had the audacity to ridicule me about not driving 3-/2 hours one way on a daily basis to participate in this trial. I haven't been back since.

My son received 4 shots in one day when he was 6 months old. I even helped hold him down for it. It makes me sick. I'll live with this guilt for the rest of my life for being so stupid not to educate myself on what was being injected into my son. He had received all of his shots except for the last required MMR. He still hasn't got that one yet and he won't ever if I can help it.

I haven't had much problem with the school about the MMR though. I just signed a waiver. I think the secretary saw the look on my face when she told me my boy needed the MMR to attend school and got scared.

A document like this should be made for for the schools to sign when they require immunizations before allowing your kids into school.

I love the form. I just wish you had posted this article 2 weeks when I took my 5 year old for his annual check up and I refused to give him more vaccinations. I would have loved to give him this form to sign. The pediatrician got really mad at me, but I just stood on my ground. A week later I got a religious exemption from my state, Florida, no questions asked.

Both of my children were damaged neurologically from vaccines. My son has severe ADHD and LD's..he was given 9 vaccines on 1 day by the same incompetent pediatrician that overdosed my infant daughter with 3 adult flu shots in 8 weeks at age 17 months old. She went from calm and focused to bouncing off the walls overnight after the 3rd flu shot. She developed eczema, food allergies and hyperactivity/attention deficit she never had before.

She was given the adult FLUZONE with high dose thimerosal instead of the mercury free version. They ran out of the pediatric ones and I was never told. She also received 3 instead of 1 (they told me one was a Prevnar and it was a flu shot by mistake) and was double dosed twice with 0.5 ml instead of 0.25 mi she was supposed to get. She stopped talking for 9 months and we spent $17,000 on her treatments so far. She took 2 years to sleep through the night after the overdose. She still doesn't go to sleep until 11 or MN every night. We are spending a fortune on a gluten free, dairy free diet. I had to quit my job and homeschool her to get her caught up after losing 20 IQ points. The doctor denied giving her 3 vaccines even though it was clearly stated in her medical records. I didn't realize my son got 9 vaccines at once back in 1992. She told me he was going to get 3 shots..she didn't tell me they combined 9 vaccines into those 3 shots. His brain never had a chance. He is a senior in high school with a 7th grade reading level.

As long as we keep our daughter gluten free her hyperactivity is better. I am proud to say that at age 5 she is doing 1st grade math and is reading. No parent should have to go through watching their neurotypical child change overnight. No parent should have to work so hard and lose the joy of childhood.

Constant worry and stress. Thanks to the lovely drug companies and the CDC and FDA.

when i told my childrens doctor that i didn't want them to get anymore vaccinations her office stoped calling my childern in for check ups and yearly wellness appointments i didn't hear from them for over 3 years i had to call them when my son needed glasses.

My girls have never been vaccinated. They are very healthy and intelligent. They have had chicken pox and just getting over whooping cough. Funny, everything they get is from families that have been vaccinated. We homeschool, but do have the proper paperwork and they can go to school if and when I want them to spend 8 hours a day socializing instead of learning, then coming home with 3 hours of homework.
