Indiscriminate use of antibiotics to fight the ulcer-causing bacterium Helicobacter pylori is behind an alarming rise in drug-resistant strains of the bug, according to a leading German microbiologist.
Dr. Manfred Kist, of the Institute of Medical Microbiology and Hygiene at the University of Freiburg, stated that gastroenterologists often prescribe antibiotics to treat patients with H. pylori without gathering enough pretreatment information.
"That is blind therapy," said Kist, who also heads Germany's National Reference Center for Helicobacter pylori.
As long as patients are treated unsuccessfully with antibiotics, the percentage of people carrying resistant H. pylori will continue to rise.
At the end of the 1980s, around 30% of patients were resistant to metronidazole, one of the three main antibiotics used in Germany to treat H. pylori infection, Kist said. Now, some 40% to 50% of patients are resistant.
Clarithromycin and amoxicillin are the other two drugs commonly used in Germany to treat H. pylori infection.
Ideally, gastroenterologists should take a biopsy of the patient's stomach tissue, so a microbiologist can test resident H. pylori for sensitivity to various antibiotics, he said. Doctors could then better determine which antibiotics would be most effective in treatment.
Currently, some 20 million to 30 million Germans are infected with H. pylori, and of those, 4 million to 6 million develop mild to severe stomach problems that require treatment, he said.
Kist is currently leading a major national research and educational project. The Robert Koch Institute -- similar to the US Centers for Disease Control and Prevention -- is participating.

I was treating patients for H. pylori 16 years ago, shortly after I read of the work of the pioneer Australian family physician, Dr. Barry Marshall who made one of the most remarkable additions to Western World medicine in the last few decades.
It is so rare that one independent physician not connected with any research institution can turn the entire medical world's view around on the cause of one of the most common medical problems we face, peptic ulcers.
Unfortunately it took Western medicine over one decade before most physicians reluctantly abandoned their concepts of what the cause of ulcers were. This is a very typical pattern with new medical information.
The amazing aspect of the treatment of peptic ulcer disease is that the drugs used to treat them (H2 blockers) were the most widely prescribed drugs in the world. They still are very commonly used.
I strongly believe that 99% of them are unnecessary and actually harm people. It was a major tragedy that these drugs became available over the counter a few years ago.
The answer is not antibiotics to treat the H. pylori infection. I used to do that from 1985 to 1993 and it frequently worked. However, the infection is really just a symptom it is NOT the true cause.
As the article above documents, relying on antibiotics to treat the problem will ultimately only make the problem worse, as eventually resistance to the drug develops.
Clearly, bacterial infections are a major cause of disease. The confusion many people have, including health care professionals, is that the bacteria are responsible for the disease.
Nothing could be further from the truth. One's weakened immune system that opens the door for these infections to come in is usually the causal factor. This is actually good news. If we had to focus our resources on avoiding bacteria we could only be partially successful.
Assuming one is not taking anti inflammatory medication like Advil or Motrin then most all of the cause is related to an imbalance of the proper bacterial growth in the intestinal system. This is easily repaired with the use of 12 glasses of filtered or bottled water per day. One should also stop all milk, juice and coffee.
Additionally, acidophilus powder one teaspoon two to three times a day will also help. My experience is that 95% of people respond favorably to this strategy if they can also stop all sugar and restrict all their fluids to water only.
We can also change our eating and work on the emotional wounding that most of us have, we can normalize our adrenal hormones and radically improve our immune system so we can prevent future infections and recover from any current ones.
The unfortunate and complicating issue in most cases is that once the "horse is out of the barn" and one has a serious bacterial infection, it frequently is necessary to use antibiotics to recover from the illness.
I fully believe that most people die from heart disease due to unresolved stress rather than their cholesterol levels or smoking.
